Please arrive ten minutes before your first appointment for efficient use of time during the consult.
Our patient registration form is available via the link below. Please complete the form and submit it and bring along copies of any recent blood test results , x-rays and scan reports, and copies of any specialist letters that you think may be helpful or relevant. This can save valuable time at the initial assessment stage and avoid duplication of testing. You can also inform us of the name of the lab where blood tests were sent and we can arrange for the results to be downloaded ahead of your initial consult.
Initial consults for new patients ensure a detailed history and health assessment is done. Please note that fees take into account the complexity of the consult plus time spent outside the consult to order/review previous history notes and test results provided plus the new tests are ordered after the initial consult to decide on the management protocols.
As a courtesy , we confirm all appointments by phone or SMS or email upto a week in advance. Please make sure that you confirm/cancel your appointment promptly to avoid a cancellation fee.

FEES
DR CHHABRA:
Initial consultation : $400 (Medicare rebate $79.70- $117.40 )
Standard Follow Up consult: $275 (Medicare rebate $79.70 )
Short Follow Up Consult: $180 (Medicare rebate $41.20)
Doctors have a legal obligation regarding repeat prescriptions and referrals; usually new referrals and scripts and requests for blood tests cannot be issued without a short Telehealth consultation.
Note: Telehealth consultations are also charged as in person visits with similar Medicare rebates if Medicare eligibility criteria are met. Medicare rebate goes into your bank account within 72 hours.

CANCELLATION policy:
A $100 fee will apply if appointments are cancelled with less than two business days notice.
